If your address has changed, please fill out the above Change of Address Form and turn it
into the Registrar's office with a copy of your deed or lease, a current utility bill, and your
driver's license reflecting the updated address.
If you and your student are currently residing with another family you will need to complete
the above Dual Residency Form. This form must be filled out by BOTH the Parent and the
Homeowner and BOTH signatures notarized. You will need to submit this form to the Registrar's office with a
copy of the Homeowner's deed or lease, original utility bill, and driver's license for both the Parent and homeowner reflecting the new/current address.
Please Note: If the parent/legal guardian owns a home in FBISD, the student must
attend the zoned school of the parent's/legal guardian's FBISD address.
If you are enrolling a student a student and you are neither the parent nor legal guardian, you
will need to complete the above Power of Attorney Form. This form must be filled out
by both the Parent and the Appointee and notarized. You will need to provide the students
grades, attendance, and discipline record from their previous school when submitting this
form to the Registrar's office for approval. The Power of Attorney must be approved by
the Principal
BEFORE the registration process can begin.
Please Note: If the parent/legal guardian lives in FBISD, the student must attend the
zoned school of the parent's/legal guardian's FBISD address.
