• Change of Address Form

    If your address has changed, please fill out the above Change of Address Form and turn it
    into the Registrar's office with a copy of your deed or lease, a current utility bill, and your
    driver's license.
     
    If you and your student are currently residing with another family you will need to complete
    the above Dual Residency Form.  This form should be filled out by both the Parent and the
    Homeowner and notarized.  You will need to submit this form to the Registrar's office with a
    copy of the Homeowner's deed or lease, utility bill, and driver's license, as well as the Parent's
    driver's license reflecting the new address.
     
    Please Note: If the parent/legal guardian owns a home in FBISD, the student must
    attend the zoned school of the parent's/legal guardian's FBISD address.
     
    If your student is planning to withdrawal from Travis High School, please complete the
    above Intent to Withdrawal Form and bring it to the Registrar's Office to complete the
    withdrawal process.
     
    If you are enrolling a student a student that you are not the parent or legal guardian of you
    will need to complete the above Power of Attorney Form.  This form should be filled out
    by both the Parent and the Appointee and notarized.  You will need to provide the students
    grades, attendance, and discipline record from their previous school when submitting this
    form to the Registrar's office for approval.  The Power of Attorney must be approved by
    the Principal before the registration process can begin.
     
    Please Note: If the parent/legal guardian lives in FBISD, the student must attend the
    zoned school of the parent's/legal guardian's FBISD address.
     
    If you would like to access the Fort Bend ISD Skyward account of your student(s), please
    complete the above Request for Skyward Form and bring it to the Registrar's office with a
    valid photo ID. Access to Skyward will allow you to view your student's grades, attendance,
    GPA, and rank throughout the school year.
     
    If you would like to change your information in Skyward, please complete the above
    Change of Information Request Form and bring it to the Registrar's office with a valid photo
    ID.  This form may be used to update your phone numbers and/or e-mail addresses only.
     

    Transcript Requests

     
    Current Students and 2015 Graduates should request their transcripts via Naviance.
     
    Graduates and/or Former Students may request their transcripts via the Transcript page.

    Verification of Enrollment (VOE)

     
    Verification of Enrollments are available year-round from the Registrar's office.
     
    During the school year, the student may request a VOE on the clipboard at the
    Registrar's office during the school day. The VOE will be ready for pick-up the next
    business day after 7th period in the black box at the Registrar's office. A VOE issued
    during the school year will expire 30 days from the date of issuance.
     
    During the summer, the student or parent may stop by the school Monday-Thursday
    8:00am-4:00pm (closed from 12:00pm-1:00pm for lunch) to sign-up for the VOE and
    pick it up the following day after lunch. A VOE issued during the summer will expire
    on the first day of the following school year.
