Each permission slip has an attached itinerary. The itinerary must be read in its entirety before submitting the digital permission form.

Every student must have a current medical and handbook form on file with the Fort Settlement MS Orchestra. If your student's medical information has changed, a new medical emergency form must be submitted.

All permission slips must be completed by the student's parents/legal guardian. Students are not permitted to complete any digital permission slips.
