Lake Olympia Middle School

Withdrawal Process:

1. Fill out and sign an INTENT TO WITHDRAW form  
2. Communicate with the Campus Registrar via email, of your students' withdrawal.  
3. Return INTENT TO WITHDRAW form along with a photo of Guardian DL/ID 

Upon completing these steps, the student will then be withdrawn from Lake Olympia Middle School.
