Clinic Guidelines for Medications & Treatments at School
- Students are NOT allowed to carry any medications (prescription -OR- over-the-counter) on them at school
- The exception is for certain emergency medications (i.e. for diabetes, asthma or life-threatening allergies) BUT the proper forms listed below must be submitted to the clinic first.
OVER-THE-COUNTER MEDICATIONS:
- OTC Medications must be delivered to the clinic by the parent/guardian in the original package/container
- Submit the PARENT-PHYSICIAN PERMIT TO ADMINISTER MEDICATION AT SCHOOL form. Parent signature is required for all OTC med forms.
- Doctor's signature will be required IF the dose indicated to be given is more then the manufacturer's recommended amount for your child and/or once the OTC med has been given more then 15 times in the school year.
PRESCRIPTION MEDICATIONS:
- Must be delivered to the clinic by the parent/guardian in the pharmacy labeled container
- If medication is taken at home as well, a second labeled container can be requested from your pharmacy for the purpose of leaving at school.
- Submit the PARENT-PHYSICIAN PERMIT TO ADMINISTER MEDICATION AT SCHOOL form. All forms for prescription and emergency meds require the parent AND doctor’s signature.
EMERGENCY MEDICATIONS:
- The nurses will administer all medications in the clinic with the exception of students that are required by their physician to carry inhalers or epinephrine pen with them at all times. In those situations, the PARENT-PHYSICIAN PERMIT TO CARRY ASTHMA AND ANAPHYLAXIS EMERGENCY MEDICATION form must be signed by the parent & doctor. as well as the appropriate form indicated below.
- If your child needs emergency meds (i.e inhaler, epinephrine pen or seizure meds) then the appropriate Action Plan AND Questionnaire listed below must also be completed/signed by the parent AND medical provider.
DIABETIC STUDENTS:
- SELF-MANAGE/SELF-CARRY: The Texas Health and Safety code allows students who can demonstrate their ability to appropriately monitor and treat their diabetes to transport their own medications and supplies and to manage their diabetes independent of the school nurse or other school district staff. To allow for this the following forms must be completed and returned to the clinic

- Please submit a copy of the Diabetic School Orders -OR- use this Diabetic Order form
- It is strongly recommended you leave an extra set of diabetic supplies, snacks or emergency meds in the clinic as back-up.
- They will all be placed in a storage container in the nurse's office, labeled with your child's name, for safe and quick access as needed
- ALL OTHER DIABETIC STUDENTS:
- Submit a copy of the Diabetic School Orders -OR- use this Diabetic Order form
- Bring all necessary supplies to clinic prior to the start of classes (all Diabetic supplies, medications and snacks)
- They will all be placed in a storage container in the nurse's office, labeled with your child's name, for safe and quick access daily and as needed.
REFILLS ON ALL MEDICATIONS/SUPPLIES ARE THE RESPONSIBILITY OF THE PARENT/GUARDIAN.
ONE REMINDER EMAIL WILL BE SENT, TO THE ADDRESS IN SKYWARD, WHEN THE QUANTITY IS LOW.
* When medication is discontinued, expired or at the end of the school year, it must be picked up by the parent/guardian or another adult which the parent/guardian designate via signed letter or email. Medications that are not picked up by the last day of school will be discarded by the clinic on the last day. No medications can be kept in the clinic during the summer.
