• Clinic Guidelines for Medications & Treatments at School

    • Students are NOT allowed to carry any medications (prescription or over-the-counter) with them to school.
      • The exception is for certain emergency medications (i.e., for diabetes, asthma, or life-threatening allergies), BUT the proper forms listed below must be submitted to the clinic first. 

     

    OVER-THE-COUNTER MEDICATIONS:

     

    PRESCRIPTION MEDICATIONS

    • Must be delivered to the clinic by the parent/guardian in a pharmacy-labeled container.
      • If medication is taken at home as well, a second labeled container can be requested from your pharmacy for the purpose of leaving at school.
    • Submit the PARENT-PHYSICIAN PERMIT TO ADMINISTER MEDICATION AT SCHOOL form. All forms for prescription and emergency meds require the parent's AND doctor's signature.

     

    EMERGENCY MEDICATIONS

    DIABETIC STUDENTS: 

    • SELF_MANAGE/SELF-CARRY: The Texas Health and Safety code allows students who can demonstrate their ability to appropriately monitor and treat their diabetes to transport their own medications and supplies and to manage their diabetes independently of the school nurse or other school district staff. To allow for this, the following forms must be completed and returned to the clinic.
    • ALL OTHER DIABETIC STUDENTS: 
      • Submit a copy of the Diabetic School Orders- OR use this Diabetic Order form
      • Bring all necessary supplies to the clinic prior to the start of classes (all Diabetic supplies, medications, and snacks)
        • They will all be placed in a storage container in the nurse's office, labeled with your child's name, for safe and quick access as needed. 

     

    REFILLS ON ALL MEDICATIONS/SUPPLIES ARE THE RESPONSIBILITY OF THE PARENT/GUARDIAN.

    ONE REMINDER EMAIL WILL BE SENT TO THE ADDRESS IN SKYWARD WHEN THE QUANTITY IS LOW. 


    * When medication is discontinued, expired, or at the end of the school year, it must be picked up by the parent/guardian or another adult whom the parent/guardian designates via a signed letter or email.  Medications that are not picked up by the last day of school will be discarded by the clinic on the last day.    No medications can be kept in the clinic during the summer.
