Every Cadet coming into the program needs to have a physical done prior to entering the class. Please hand into to course instructors.

* Get this form signed by the instructor - not parent - unless your volunteering is performed with your parent

* The Cadet needs to make a copy for themselves so that they are keeping track of their own hours, and hand this into MCJROTC and any other organisation that they need hours for.

In order for a cadet to attend an out of school activity - they need this permission slip to be signed by all teachers
