The following forms MUST be submitted by students trying out for any sport in FBISD:

• Athletic Participation
• Emergency Contact
• Medical History

As per UIL policy, all students must complete the following steps to be eligible for sports participation in the 2025-2026 school year. Physicals must be uploaded into RankOne.com.

Please review the key details below:

• Physical Requirement: All student-athletes & pre-athletic PE students (except in certain cheer situations) must have a physical dated after April 1, 2025 to participate next school year.
• Expiration: Current physicals from the 2024-2025 school year will expire on May 31, 2025.
• Rank One Electronic Forms: Available starting April 1, 2025.

We encourage all students to complete their physicals before summer break to avoid delays in participation.
