The Child Nutrition Dept. can offer dietary accommodations for studentds at no extra cost for students or families when it is medically necessary. The request forms must be signed by a physician or recognized medical authority and submitted to the Child Nutrition Dept. by Fax: 281-634-1862 Attn: Dietician OR by Email: DLCNDSpecialDiet@fortbendisd.com. A new request for dietary accommodations form does not need to be submitted each school year unless there are any changes. Dietary accommodations may take up to 2-4 weeks to process, especially at the beginning of a school year. Please note, this form is different for the allergy forms collected by Student Health Services.
