Student Clinic Visits

When your student visits the clinic, they will be sent home with a copy of the Teacher’s Referral to Nurse form.

This form includes:

• Time your student came to the clinic

• Time they left the clinic

• Reason for the visit

• Exam findings

• Treatment provided

This process helps keep parents informed about their child’s clinic visits.
