School Screenings

Each school year, students in Kindergarten, 1st, 3rd, and 5th grades participate in hearing and vision screenings.

If your child brings home a referral form for a follow-up exam:

1. Please take the form to your healthcare provider for evaluation.

2. Return the completed form to the school nurse.
