If you would like to do your University or ACP observation hours in Fort Bend ISD, please read the following instructions:
We will schedule observations hours from September 15 - November 28, 2008; January 12 - March 27, 2009 in the following areas of certification only:
Please complete a
Criminal History
Form (link will be active in September 2008).
On the form (Requires

), please list two to three names of campuses
where you would like to conduct your observation hours. After you complete
the form, hit the 'submit by e-mail' button toward the bottom of the form.
You do not need to complete this form if you have an online application on
file. If you have any problems submitting the form via e-mail, you can
fax it 281.327.1280. (see Disclosure below)
Submit your University letter or ACP acceptance letter to the Human Resources Office (ATTN: Yaffa). Our office is located at 16431 Lexington Blvd., Sugar Land, TX 74479. You can mail, e-mail or fax the letter to us - 281.327.1280. (Applicants who submit their ACP letter and criminal history form together are scheduled first.)
E-mail Yaffa (Yaffa@fortbendisd.com) after both documents have been submitted and received in our office. Criminal histories are not run until we receive your ACP or University Letter. Do not contact the principal or teachers until you receive your confirmation e-mail from Human Resources.
We will respond to your e-mail to let you know your campus assignment and contact person (Please allow 7-10 days after we receive your documents, e-mail and run your criminal history.). It is your responsibility to then contact them to schedule your observation days/time.
Disclosure to Employment Applicant Regarding Procurement of a Consumer Report
| In connection with your application for employment, we may procure, or cause to be procured, a consumer report (excluding credit information, but including public record information), on you as part of the process of considering your candidacy or status as an employee or volunteer with the School District (ISD). The ISD will use sources including, but not limited to, NCTC, 24 Briercroft Office Park, Lubbock, Texas 79412 and the Texas Department of Public Safety to procure criminal history information. In the event that information from a report is utilized in whole or in part in making an adverse decision with regard to your status as an employee, the ISD will provide you with a copy of the consumer report, as allowed by law, and a written description of your rights under the law. |
