When registering your child, the following items are needed:
1) Proof of Birth (the original Birth Certificate).
2) Immunization Records.
3) Proof of Residency in the parent's name with a current utility bill (i.e., light, water, or gas bill with the parent's name and address on the bill). Telephone bills are not acceptable.
4) Copy of the current lease or deed.
5) Your child's Social Security Card.
6) Picture ID of Parent/Guardian (i.e. driver's license with current address).
7) Recent Report Card or Grade Transcript.
