All sixth grade students who will be entering seventh grade in the school year 2009-2010 will be required to show proof of having received the following three vaccines:

1. The Tdap (tetanus, diphtheria and acellular pertussis)
2. The meningococcal vaccine
3. A second varicella vaccine (As in the past, a written statement from the parent or legal guardian or physician documenting a child’s positive history of varicella disease (chickenpox) is acceptable in lieu of either dose of varicella vaccine.)
