Please remember if you have received a health screening referral from the Clinic to help us keep our records updated by providing a copy of the physician report to the Clinic. If you require any assistance or further information, please feel free to contact me.
 
Attention 8th Grader Parents: Most students in 8th grade will need a DPT, Td or Tetanus booster before 9th grade. This is due every 10 years and usually the last was received when they started school. Please take a look at your child's immunization record. I will also send out reminders later in the Semester. If your child has already received this or any other updated immunizations, please provide us a copy of the record. Remember, current immunizations are a State requirement.
